LPN Licensed Practical Nurse for Personal Care Evening Shift

About the role

Deer Meadows Personal Care Home is seeking a Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents.

Responsibilities

• Providing high-quality nursing care to residents in our Personal Care Home
• Developing and implementing individualized care plans in collaboration with the interdisciplinary care team
• Maintaining accurate and complete records of resident care, including documentation of resident interactions
• Monitoring and reporting changes in resident condition, including any concerns or issues that may arise
• Education residents, families, and caregivers on care instructions

Requirements

• Must be a Licensed Practical Nurse (LPN) with a current license to practice in the state of Pennsylvania
• Prior experience in a healthcare setting
• Effective in a fast-paced environment, prioritizing tasks and managing time efficiently
• Excellent communication and interpersonal skills, with the ability to work collaboratively with the interdisciplinary care team
• Compassionate and respectful attitude towards residents, families, and colleagues
