Licensed Practical Nurse (LPN) positions available in full-time, part-time, and per diem capacities at Elderwood at Hornell, a highly rated skilled nursing community.

About the role

Two career-building options are available:

  • Core Team (Full-Time & Part-Time): Join the Elderwood at Hornell team for stability, growth, and meaningful resident relationships. Work alongside a supportive leadership team and play an essential role in providing exceptional care.
  • Per Diem (ESS): Maintain flexibility and control through Elderwood Staffing Solutions (ESS), an internal staffing program for experienced clinicians. Pick up shifts based on availability and gain exposure to multiple clinical settings across Western New York, Central New York, and the Adirondacks.

Responsibilities

  • Administer medications and treatments to assigned residents as ordered by a physician, adhering to the five rights of medication administration.
  • Direct and monitor personal care duties and nursing care procedures carried out by Nursing Assistants on the assigned team.
  • Assist Nursing Assistants with direct resident care as necessary.
  • Report unusual symptoms, changes in condition, daily needs, and resident progress to the Unit Manager, Assistant Unit Manager, or Charge Nurse Supervisor.
  • Follow the Plan of Care for each resident and monitor Nursing Assistants' implementation of the care plan.
  • Receive, store, and distribute controlled substances, ensuring proper and safe placement in medication carts, stock cupboards, medication room refrigerators, and narcotic cabinets.
  • Document care in care plans, Nursing Notes, Medical Administration Records, Treatment Records, and other required records.

Requirements

  • Current state license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
  • LPN training and work experience in a geriatric setting preferred.
  • Ability to supervise and collaborate effectively with other personnel.
  • Strong verbal and written communication skills.
  • Caring, respectful attitude toward residents.
  • Physical stamina for constant activity.
  • Ability to perform tasks to established standards of excellence.
  • Intermediate or advanced proficiency in reading, writing, speaking, and understanding English to ensure safe communication with residents, coworkers, and visitors.

Pay

Base pay: $28.00 – $35.54 per hour, based on experience.

  • Evening Shift (2nd Shift) Differential: $2.00/hour (weekday), $3.00/hour (weekend).
  • Night Shift (3rd Shift) Differential: $1.50/hour (weekday), $2.50/hour (weekend).

Schedule

  • Days: 6:00 AM – 2:00 PM, 6:00 AM – 6:00 PM.
  • Evenings: 2:00 PM – 10:00 PM.
  • Overnights: 10:00 PM – 6:00 AM, 6:00 PM – 6:00 AM.
  • Includes every other weekend requirement.
