About The Role
As an LPN License Practical Nurse at Tucker House Nursing and Rehabilitation, you will be responsible for:
- Providing direct patient care to our residents, including medication administration, vital sign monitoring, and wound care
- Assisting with daily living activities, such as bathing, dressing, and grooming
- Developing and implementing individualized care plans in collaboration with our interdisciplinary care team
- Maintaining accurate and detailed records of patient care and treatment
- Participating in quality improvement initiatives to ensure the highest standards of care and service
- Collaborating with other healthcare professionals to ensure comprehensive care and support
Requirements
- Current LPN License in the State of Pennsylvania
- High school diploma or equivalent required
- Previous experience in long-term care or post-acute rehabilitation preferred
