Responsibilities

  • Provide direct patient care under the supervision of a registered nurse.
  • Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
  • Implement the plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participate in coordination of home health and hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
  • Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.
  • Participate in personal and professional growth and development.
  • Perform other duties assigned by the registered nurse, including caring for hospice patients as census allows.
  • Participate in weekend and holiday on-call rotation requirements.

Qualifications

  • Must be a graduate from an accredited practical nurse or vocational nursing program.
  • Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio.
  • 3 years of nursing experience, preferred.
  • Prior experience in community/home health and/or medical/surgical, preferred.
  • Comply with accepted professional standards and practices.
  • Demonstrates good verbal and written communication, and organizational skills.
  • Must be able to lift, position or transfer patients and lift supplies and equipment.

Pay

$28.00 - $31.00 per Hour

Schedule

Monday - Friday; Standard Business Hours & Occasional On-Call Rotation
