What You'll Help With

Transform Healthcare's 20-bed telemetry PCU specializes in the care and management of all general and bariatric surgery patients. This unit houses the majority of Enhanced Recovery (ERAS) patients and serves as the alternate unit for neuro/trauma surgery patients, hospitalist/medicine patients, and urology/other surgery patients.

Responsibilities

• Provides practical nursing care under the direction of a registered nurse or physician.
• Collaborates with the healthcare team to deliver safe, effective, and patient-centered care across the inpatient setting.
• Develops and implements individualized interdisciplinary plans of care.
• Identifies and addresses patient, family, and caregiver educational needs.
• Administers medications in accordance with provider orders and scope of practice.
• Applies knowledge of growth and development to deliver age-appropriate care.
• Advocates for patients and supports their rights, providing psychological, social, and cultural support.
• Collaborates with interdisciplinary team members to ensure coordinated and effective patient care.
• Supports performance improvement initiatives and contributes to quality outcomes.
• Participates in orientation and development of new staff.
• Demonstrates professionalism, teamwork, and commitment to service excellence.

Requirements

• Graduate of an accredited school of practical nursing.
• Current licensure as a Licensed Practical Nurse (LPN) in the Commonwealth of Virginia.
• AHA BLS-HCP certification.

Qualifications

• Must successfully complete orientation and competency requirements.
• Must be able to meet essential job functions.
