Responsibilities
- Provide nursing care under the supervision of a Hospice RN to support the comfort, health, and dignity of patients and families, following the interdisciplinary plan of care
- Assess physical, emotional, social, and spiritual needs, collaborating with the care team to coordinate individualized support and services
- Maintain accurate, up-to-date documentation that reflects patient status, interventions, and goals, ensuring compliance with hospice protocols and regulations
- Educate patients and caregivers based on their condition and care needs, promoting safe and effective caregiving at home
- Participate in care team meetings, training, and special projects while reporting any concerning issues to the RN or Clinical Director
Qualifications & Requirements
- Must be a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the practicing state(s) with a current CPR certification
- Requires previous experience in hospice, home health, medical-surgical, or patient-specific care such as geriatrics or pediatrics
- Must have a reliable vehicle, valid driver's license, and state-required auto liability insurance
- Must demonstrate a strong commitment to the organization's mission, values, and philosophy of care
- Physical demands include frequent walking, standing, sitting, kneeling, reaching, and lifting up to 200 lbs with assistance; regular use of hands for grasping, handling, and documentation
- Work environment may include exposure to temperature extremes, indoor/outdoor conditions, stairs, moderate noise, computer use, and occasional contact with chemicals, fumes, and bodily fluids
Pay & Schedule
- Salary: $30.00 - $36.00 an hour
- Schedule: 3/12s Nights 7:00pm - 7:30am
