LPN Hospice Field

About the role

As a member of Empath Health, Tidewell Hospice provides compassionate, dignified care to patients with advanced illness and their families in the coverage area spanning from North Sarasota to Venice/Nokomis.

Responsibilities

• Identify patient/family needs and coordinate interdisciplinary team provision of services under physician's orders.
• Utilize monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
• Work under the direction of the Primary RN and collaborate with Primary RN and/or CTM.

Requirements

• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
• Licensure: Valid Florida LPN License.
• Driver’s License: Valid Florida State Driver’s License.
• Continuing Education: As required for licensure.
• Certifications: Valid CPR certification from the American Red Cross or American Heart Association; Current IV Therapy certification preferred. Must obtain the certification within six months of hire date.

Qualifications

• Must be comfortable working in a highly mobile, field-based position.
• Experience providing skilled nursing care such as wound care, Foley catheter insertions, and assisting with patient needs including bathing, feeding, and ADLs as required.

Skills

• Strong communication and interpersonal skills.
• Ability to work independently and as part of an interdisciplinary team.
• Knowledge of medical terminology and procedures.
