About the Role
Licensed Practical Nurse (LPN) providing skilled nursing care in patients' homes for pediatric and adult clients.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services in accordance with prescribed orders.
- Monitor signs and symptoms indicating physiological and psychosocial changes.
- Collect, analyze, and interpret healthcare data; document nursing diagnoses.
- Provide quality patient care, assess health status, and collaborate with healthcare team members to ensure optimal patient outcomes.
Requirements
- Current LPN/LVN license in the practicing state.
- Valid TB or Chest X-Ray and BLS certification.
- Preferably one year of professional nursing experience.
