LPN Homecare - Full-Time Nights - PRN Weekends

About the role

The Licensed Practical Nurse (LPN) provides skilled nursing care to adult patients in their homes, utilizing the nursing process to assess, plan, implement, and evaluate care.

Responsibilities

• Assess and monitor patient signs and symptoms, including physiologic and psychosocial changes.
• Administer medications and perform tasks related to G-tube care; future responsibilities may include trach/vent management.
• Document patient data and collaborate with healthcare team members to ensure quality care.
• Provide direct patient care in accordance with prescribed orders, assess patient condition, and ensure accurate documentation.

Requirements

• Current LPN/LVN license in the practicing state.
• Current TB or Chest X-Ray and BLS certification.
• Preferably, at least one year of professional nursing experience.
