About the role
Licensed Practical Nurse (LPN) providing in-home care for Pediatric and/or Adult patients with GTube, Trach, or Vent dependencies.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services in accordance with prescribed orders.
- Monitor signs and symptoms of physiological and psychosocial changes.
- Collect, analyze, and interpret data from healthcare team to identify nursing diagnoses.
Qualifications
- Current LPN/LVN license in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
- Preferably one year of professional nursing experience.
Key Duties
- Provide patient-centered care in home settings, including PRN overnights and weekend shifts.
