About the role
Licensed Practical Nurse (LPN) providing in-home care for Pediatric and Adult patients, including those with GTube, Trach, or Vent needs.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services according to prescribed orders.
- Monitor signs and symptoms indicating physiologic and psychosocial changes.
- Collect, analyze, and interpret healthcare data, documenting nursing diagnoses.
- Provide personalized nursing care in patients’ homes, ensuring safety and compliance with medical orders.
- Assess patient conditions and document findings accurately.
Qualifications
- Licensed as an LPN/LVN in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
Preferred
- One year of professional nursing experience.
