About the role
Providing in-home care for pediatric and adult patients.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services as per prescribed orders.
- Monitor signs and symptoms to detect physiological and psychosocial changes.
- Collect, analyze, and document patient data and nursing diagnoses.
Requirements
- Licensed as an LPN/LVN in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
Preferred Qualifications
- At least one year of professional nursing experience.
Additional Information
- Provide care for patients with G-tube, Trach, Vent, including adult, pediatric, and complex needs.
- Flexible shifts including weekend overnights and PRN as needed.
