About the Role
Provide in-home skilled nursing care for pediatric and/or adult patients.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services as per prescribed orders.
- Monitor and interpret signs of physiological and psychosocial changes.
- Collect and analyze health data.
- Document diagnoses and care activities.
- Collaborate with the healthcare team to ensure compliance with medical protocols and documentation standards.
Requirements
- Current LPN/LVN license in the practice state.
- TB clearance or Chest X-Ray.
- BLS certification.
- One year of professional nursing experience (preferred).
Skills & Key Duties
- Provide compassionate, skilled nursing care.
- Assess patient conditions and ensure adherence to care plans.
