About the role
Provide in-home care for pediatric and adult patients, including those with GTube, Trach, and Vent needs.
Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Deliver skilled nursing services according to prescribed orders.
- Monitor signs of physiologic and psychosocial changes.
- Document diagnoses and patient data.
Qualifications
- Valid LPN/LVN license in the state of practice.
- Current TB or Chest X-Ray.
- BLS certification.
- At least one year of professional nursing experience.
Skills
- Conduct assessments.
