About the role
Licensed Practical Nurse (LPN) providing in-home care for Pediatric and Adult patients. Assess, plan, implement, and evaluate patient care using the nursing process. Deliver skilled nursing services according to prescribed orders. Monitor signs and symptoms to identify physiologic and psychosocial changes. Collect, analyze, and document healthcare data, including potential nursing diagnoses.

Qualifications
- Licensed as an LPN/LVN in the practicing state
- Current TB or Chest X-Ray and BLS certification
- Preferred: at least one year of professional nursing experience
