LPN | Home Visits

Join our Atrio Home Care team—a partnership between Holland Home, Brio, and Rest Haven—as a compassionate Licensed Practical Nurse (LPN) providing in-home care throughout the Grand Rapids territory. This role focuses on delivering high-quality, one-on-one care to clients in their homes within approximately a 40-mile radius of Grand Rapids.

Responsibilities

• Deliver skilled nursing care under RN supervision, including wound care, dressing changes, catheter care, and medication/injection administration
• Follow and support individualized Plans of Care in collaboration with physicians and the care team
• Provide specialized nursing services such as preventive care (skin care, pressure relief) and chronic disease management
• Educate clients and caregivers on disease processes, self-care, and health promotion to encourage independence
• Maintain accurate, timely documentation using the Matrix EMR system (iPads provided)

Requirements

• Current Michigan LPN license
• Valid driver’s license, reliable transportation, and proof of auto insurance
• 1–2 years of acute care or skilled nursing experience (home health preferred)
• Passion for delivering exceptional care to older adults

Schedule

• Territory: Grand Rapids team (approximately 40-mile radius)
• Full-time or part-time opportunities available
• Day shift (8:00 AM – 5:00 PM)
• No weekend, on-call, or holiday requirements
