LPN Home Health Liaison

Help patients transition safely home as the key connection between referral sources, patients, families, and our HomeHealth team. Leverage your clinical expertise and communication skills to coordinate seamless transitions from hospital to home.

Responsibilities

• Collaborate with referral sources, discharge planners, case managers, and providers to coordinate HomeHealth services.
• Review patient information and obtain required demographics, insurance, and clinical documentation.
• Meet with patients and families to assess home care needs and support safe discharge planning.
• Obtain and maintain consents, authorizations, Advance Directive information, and physician orders.
• Coordinate with intake, scheduling, and clinical teams to ensure timely start of care.
• Educate referral sources on ChristianaCare HomeHealth services.
• Serve as a trusted resource for patients, families, and referral partners throughout the care transition process.

Requirements

• Current LPN license in the State of Delaware.
• Knowledge of HomeHealth services, practices, and care transition processes.
• Excellent communication, customer service, and relationship-building skills.
• Strong organizational, problem-solving, and computer skills.
• Ability to work independently in a fast-paced environment.
• Valid driver’s license and active auto insurance required.

Pay

Hourly pay range: $27.31 - $40.96. The actual salary within this range offered to a successful candidate will depend on individual factors including skills, relevant experience, and qualifications.

Schedule

Monday through Friday, 8:00 AM – 4:30 PM, with some 4-hour weekend days and holidays as needed by the department.
