Role Overview: Provide skilled nursing services to patients in their homes under the supervision of a Registered Nurse. Assess, plan, implement, and evaluate patient needs and care to ensure high-quality, patient-centered outcomes. Key Responsibilities Deliver personalized care across diverse patient populations in a community setting.Build strong, trusting relationships with patients and their families.Maintain accurate documentation of patient care and progress.Adhere to all safety and clinical protocols, including certifications and licensure requirements. Qualifications & Skills Licensed Practical Nurse (LPN) license valid in the state of practice.Minimum of 1 year of nursing experience.Current BLS certification through the American Heart Association.Valid driver’s license and ability to travel to patient homes.Physical ability to lift up to 50 lbs, stand, walk, and perform physical tasks required for patient care. Additional Details Participate in rotating on-call weekends and holiday rotations.Mileage reimbursement at $0.70 per mile (subject to change).Enjoy autonomy, meaningful work, and flexible scheduling.
