About the role

We are seeking a dedicated Full-Time Licensed Practical Nurse (LPN) for home health care in the Guntersville/Grant, AL area.

Responsibilities

• Provide high-quality clinical and technical nursing care within the scope of practice, following the plan of care and infection control protocols.
• Accurately document patient visits in electronic medical records and electronic visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.
• Adapt to various home environments and serve patients of diverse backgrounds.

Qualifications & Skills

• Current unrestricted LPN license in Alabama.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Preferred: CPR certification, home care experience, multitasking ability, independent work skills, strong communication, and organizational skills.
• Flexibility to meet patient needs.

Compensation & Benefits
