Duties & Responsibilities
- Provide direct nursing care to home care and hospice clients following the RN-directed plan of care.
- Carry out physician orders and communicate changes in client condition to the RN and care team.
- Document client care accurately and share relevant updates with interdisciplinary team members.
- Maintain infection control, safety standards, and proper clinical procedures.
- Support care coordination including scheduling, referrals, and client admissions as assigned.
- Collaborate with team members to ensure clients receive consistent, high-quality care.
Qualifications
- Graduate of an accredited school of nursing.
- Current LPN licensure in the State of Minnesota.
- Current, valid MN Driver's License and current auto insurance.
- Ideal Candidate Experience in home care/hospice preferred but not required.
- Compassionate caregiver committed to patient dignity and comfort.
- Strong communication and teamwork skills for collaboration with interdisciplinary care teams.
- Organized and dependable with the ability to prioritize tasks and manage multiple client needs.
- Skilled in observation, documentation, and reporting changes in patient condition.
- Knowledge of infection control, safety practices, and clinical procedures.
- Comfortable providing care in home and hospice environments with a patient- and family-centered approach.

Pay
A new team member's starting wage reflects our compensation philosophy, taking into account experience, education, certifications and internal equity. Hourly pay is just one part of the comprehensive and rewarding compensation package we're proud to provide!
Schedule
This is a full-time, 80-hour per pay period (1.0FTE) position. The schedule is Monday through Friday, 8:00am - 4:30pm. No call, weekends, or holidays.
