LPN Full Time or Part Time Evening Shift 3p-11p

About the Role

The Licensed Practical Nurse (LPN) plays an integral role in the success of our team, our community, and creating a place our residents are proud to call home. The primary purpose of this position is to provide high-quality, holistic nursing care in accordance with nursing practice standards and health care regulations that govern the practice of professional nursing. The LPN will lead or direct licensed and non-professional staff in the delivery of direct resident care and support functions.

Responsibilities

• Conduct daily nursing functions in accordance with current rules, regulations, and guidelines governing long-term care facilities/assisted living communities.
• Ensure compliance of nursing personnel with written policies and procedures, including the safety program.
• Admit, transfer, and discharge residents with documentation regarding care and services provided.
• Participate in orienting new residents/family members to the facility.
• Make rounds with healthcare providers, Hospice staff, and Home Health staff as needed.
• Utilize nursing personnel qualifications according to the resident's care plan, service plans, and healthcare provider orders.
• Complete daily clinical and environmental rounds to evaluate resident care and report significant events.
• Meet and resolve issues with residents and/or family members; report significant issues to the supervisor.
• Participate in facility surveys (inspections) as requested by management.
• Manage clinical programs as directed (e.g., dementia, wound care, quality assurance, infection control, admissions, and falls).
• Complete initial assessments and assist in developing interim care plans/service plans.
• Update, review, and transcribe care plans based on nursing needs and healthcare provider orders.
• Communicate with healthcare providers to review treatment plans, complete orders, and progress notes.
• Review resident medical records for specific treatments, medication orders, diets, labs, etc.
• Requisition and arrange diagnostic and therapeutic services as ordered by the healthcare provider.
• Communicate department policies and procedures to personnel, residents, and visitors.
• Provide written and oral reports/recommendations concerning resident condition and shift activities.
• Complete accident/incident reports involving residents and submit them to the Director of Nursing.
• Provide resident and family information for care conferences or other meetings.
• Assist in arranging transportation for residents and other third-party services.
• Prepare and administer medications ordered by the healthcare provider.
• Ensure availability of resident medications and maintain an adequate supply of floor stock medications, supplies, and equipment.
• Review medication administration records for completeness and accuracy.
• Dispose of drugs and narcotics according to established procedures.
• Supervise the nursing unit to ensure staff adherence to nursing standards.
• Work with the Director of Nursing to resolve staff performance issues.
• Assure availability of protective equipment for staff.
• Maintain cleanliness and sanitation in nursing service work areas, resident rooms, and treatment areas.
• Recommend equipment and supply needs to the Director of Nursing.

Qualifications

• Must meet all applicable state and federal requirements for this position.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in the applicable state, in good standing throughout employment with no disciplinary action in effect against the professional license.
• Prefer one year of experience in nursing in a home care, assisted living, or long-term care facility.

Working Conditions and Physical Demands

Tasks may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals. Physical requirements include walking long distances, standing for extended periods, sustaining repetitive movements, lifting up to fifty pounds, and pushing/pulling up to fifty pounds. Cognitive requirements include independent task execution, clear communication, and general computer literacy.

Schedule

Evening shift: 3:00 PM - 11:00 PM, every other weekend required.

Pay

$1 evening shift differential, $2 weekend shift differential.
