About the role
Provide personalized, skilled nursing care in home settings, including medication administration, treatments, trach/vent care, G-tube management, monitoring patient status, and educating patients and families for long-term success.
Responsibilities
- Deliver direct patient care across pediatric, adult, or geriatric cases
- Coordinate with care teams and monitor patient health
- Perform skilled interventions based on case needs
- Build meaningful patient relationships and ensure quality care
Requirements
- Valid LPN license
- Current CPR certification
- Strong clinical and communication skills
- Reliable transportation
- Preferred 6 months to 1 year of nursing experience; new graduates welcome with training and mentorship provided
