About the role
The LPN at Harmony provides quality nursing care and services to residents in a respectful and dignified manner, coordinating physical, social, and emotional aspects of assisted living care and services in accordance with the physician's orders and the resident's choices.
Responsibilities
- Execute the medication management plan including periodic review of the documentation, observation of the effectiveness of the plan, and communication to the physician when changes are needed or outcomes are not appropriate
- Continuously assess resident needs with updates to appropriate individuals as it relates to care, services, and changes in condition
- Implement measures to ensure resident wellness and safety and document those measures appropriately
- Work in a team fashion to meet resident needs
- Respond to emergencies timely and in a calm and competent manner
- Report incidents and changes in resident condition to the Healthcare Director and with appropriate documentation
- Be an active participant in all activities including community events
- Maintain the dining room, monitor resident diet acceptance, and ensure adequate nutrition
- Maintain high standards with regard to record keeping, resident and staff documentation
- Establish and maintain a positive working relationship with all departments
- Identify risk factors for residents and initiate interventions including appropriate documentation
- Provide a thorough report of resident status to the on-coming nursing team, at each change of shift, and in the Daily Log
- Motivate and direct CNA/RMA's
Requirements
- Current LPN license to practice nursing in the state in which the community is located, holding a license in good standing with the Board of Nursing
- Experience working with the senior population, preferably in assisted living or LTC
- Team player who works well with others and has experience supervising staff
- Strong critical thinking skills, effective verbal and written communication abilities
- Demonstrated ability to manage time efficiently with strong organizational skills
- In good physical condition and capable of sitting, standing, bending, kneeling, walking, and lifting up to 50 lbs
- Willingness to work an 8-16 hour flexible schedule to include weekends and holidays
- Attendance at all required trainings, CEU's, and staff meetings
- Certified or willing to take and pass the certification class required by the state in which employed
