About the role
Enhabit Home Health & Hospice, one of the nation’s largest home‑based care providers, is seeking a Licensed Practical Nurse to deliver compassionate care in both home health and hospice settings. Responsibilities
- Take and record patients' vital signs (weight, temperature, blood pressure, pulse, respiration).
- Prepare and administer injections.
- Monitor catheters and dress wounds.
- Assist with basic care tasks such as bathing, dressing, personal hygiene, standing, walking, and eating.
- Communicate compassionately with patients and families.
- Observe patients closely, uphold high ethical standards, and recognize when to alert physicians or other providers about changes in condition.
- Provide care across both home health and hospice environments, supporting patients throughout their entire care journey.
Qualifications
- Graduate of an approved school of nursing and licensed as a Licensed Practical Nurse in the state of employment.
- Valid state driver’s license, automobile liability insurance, and dependable transportation.
- Minimum of one year of clinical experience.
- Previous home health or hospice experience is preferred.
