Responsibilities
- Deliver nursing care aligned with residents' care plans
- Conduct health and psychosocial evaluations
- Monitor residents' conditions and promote health and prevention
- Coordinate services to ensure optimal resident well-being
- Provide comprehensive health assessments
- Enhance the quality of life for residents through coordinated care services
Requirements
- Licensed Practical Nurse license in good standing
- Preferred: Two years of LPN experience, especially in assisted living or nursing homes (new grads welcome)
- Strong judgment, problem-solving, and decision-making abilities
- Excellent organization, communication, and time management skills
- Ability to handle multiple priorities and delegate effectively
- Dedication to seniors' well-being and ability to work nights and weekends
- Capability to work in both Assisted Living and Memory Care settings
