About the Role

Responsible for providing direct nursing care to residents and supervising the day-to-day nursing activities performed by nursing assistants. Supervision must adhere to current Federal, State, and local standards, guidelines, regulations, and facility policies to ensure the highest degree of quality care is maintained at all times.

Schedule

• 12-hour shifts: 7am–7pm or 7pm–7am

Pay

Qualifications

• Current, unencumbered RN or LPN license in Illinois and/or Wisconsin
• Current CPR certificate or certification within two weeks of employment
• Knowledge of nursing and medical practices and procedures
• Interest and/or knowledge in geriatric care
• Ability to read, write, and speak English clearly
• Ability to make independent decisions when circumstances warrant
• Tactful and professional demeanor with residents, families, visitors, personnel, and government agencies
• Leadership and supervisory ability; willingness to collaborate with team members
• Personable, enthusiastic, and resourceful

Physical Requirements

• Ability to move intermittently throughout the workday
• Ability to cope with mental and emotional stress of the position
• Adequate sight and hearing (with or without prosthetics) to perform job requirements
• Ability to function independently, demonstrate flexibility, personal integrity, and work effectively with residents, families, personnel, and support agencies
• Good general health and emotional stability
• Ability to work with ill, disabled, elderly, emotionally upset, and occasionally hostile individuals
• Ability to lift, push, pull, and move equipment, supplies, and residents in excess of fifty (50) pounds
• Exposure to infectious waste, diseases, and conditions, including HIV and Hepatitis B viruses

Responsibilities

• Accept and support assignments as directed, including different units, residents, workloads, and multiple tasks
• Plan, develop, organize, implement, evaluate, and direct nursing staff on a respective unit
• Ensure all nursing procedures and protocols are followed according to established policies
• Supervise and instruct nursing staff on job-related topics to promote professional development
• Ensure staff adhere to departmental policies, procedures, and dress code
• Assist in developing individualized care plans for residents
• Attend and actively participate in resident care plan conferences
• Report resident or family concerns and complaints to the Administrator
• Counsel or discipline nursing personnel as necessary
• Conduct daily rounds to ensure staff perform required duties and follow appropriate procedures
• Perform daily physical rounds with residents and physician rounds as needed
• Maintain a warm, positive, and calm environment throughout the department
• Attend continuing education programs
• Recommend equipment and supply needs to the Director of Nursing
• Order medications and treatments within established budgetary guidelines
• Utilize PCC 24-hour summary for resident care recommendations and concerns
• Complete accident/incident reports thoroughly
• Encourage physicians to review treatment plans, record orders, and sign progress notes
• Chart nurses’ notes descriptively, reflecting care provided and resident responses
• Complete required assessments upon resident admission, transfer, and discharge
• Receive and record physician telephone orders on appropriate forms
• Record and forward new or changed diet orders to the Dietary Department
• Perform routine charting duties according to established policies
• Complete admission, discharge, and transfer processes
• Prepare and administer medications and treatments as ordered by physicians
• Review medication records for completeness, accuracy, and adherence to policies
• Administer professional services such as catheterization, tube feedings, suction, wound care, colostomy care, and post-mortem care as required
• Orient new residents and family members to the facility
• Arrange diagnostic and therapeutic services as ordered by physicians
• Notify attending physician and family of changes in resident condition
• Obtain and process sputum, urine, and other lab tests as ordered
• Monitor and record vital signs, blood pressures, and accuchecks as necessary
• Contact physicians for nursing assessments, medication orders, incident reports, abnormal lab/X-ray results, and other care-related matters
• Ensure nurses’ notes and monthly summaries align with care plans
• Attend staff meetings and perform other related duties as assigned
