LPN for Assisted Living & Memory Care

Position Summary

Addington Place of Clinton is seeking a Licensed Practical Nurse (LPN) to lead and coordinate the health and wellbeing of residents in their Assisted Living and Memory Care Community.

Responsibilities

• Works with the Director of Health & Wellness (DHW) and assists with assigned duties.
• Creates and posts a timely schedule for all personal care staff.
• Ensures proper staffing levels by filling call-offs and staff vacancies.
• Partners with the Director of Health & Wellness and other team members to analyze and maintain resident and team member health safety.
• Identifies ongoing needs and services of residents through assessments and Service Plans.
• Maintains confidentiality of resident information, including medication details.
• Assists/observes medications and treatments for residents, following state regulations.
• Communicates with residents' families and/or responsible parties regarding changes in care or health concerns.
• Addresses accidents, incidents, and unsafe conditions immediately, completing incident reports.
• Participates in change of shift reporting and the development of the Service Plan.

Qualifications

• Maintains current LPN license in the State of Iowa.
• Experience in assisted living, home health, or long-term care industries preferred.
• Two (2) years experience as a Licensed Nurse preferred.
• One (1) year nursing management experience preferred.
• Demonstrates competence in assessment skills, injections, medication administration, follow-up, and triage.
