About the role
Provides skilled nursing care to patients in home settings, specializing in ventilator, tracheostomy, and G/J tube care. Must be willing to work in multiple environments, including at least two different homes, with a minimum of 72 care hours per quarter.
Responsibilities
- Plan, implement, and evaluate patient care; initiate care plans; administer medications; monitor patient responses; and coordinate care with healthcare teams.
- Utilize the nursing process to deliver and assess patient care.
- Adjust care plans based on patient needs and physician instructions.
- Perform therapeutic, preventive, and rehabilitative nursing actions.
- Respond to patient condition changes and communicate with physicians or supervisors.
- Administer medications and treatments, monitor effectiveness, and document outcomes.
- Review care protocols and identify education, care coordination, and discharge planning needs.
Requirements
- Current LPN license for the practicing state.
- Valid health certificate, PPD or chest X-ray, and BLS certification.
- At least one year of LPN experience.

- Complete assigned care hours; adhere to policy regarding patient visits and maximum hours per home.
- Training hours do not count towards care hours.
