About the Role

The Clinical Care Coordinator performs patient assessments, gathers data, and implements care plans under the supervision of licensed medical staff.

Responsibilities

• Accurate clinical documentation and maintenance of patient records
• Preparation of patients for exams
• Administration of medications and vaccines
• Management of medical supplies
• Ensuring compliance with healthcare regulations
• Provision of patient education
• Follow-up on lab results and prescriptions
• Response to patient and provider communications
• Assistance with insurance pre-authorizations
• Building collaborative relationships within a multidisciplinary team to support patient care

Requirements

• Licensed Practical Nurse (LPN) with at least one year of experience preferred
• Active Virginia LPN license
• Current CPR/BLS certification
• Demonstrated clinical competency and attention to detail
• Strong communication skills
• Ability to work effectively in a team environment
