LPN - Family Practice

Role Overview: The Clinical Care Coordinator performs patient assessments, gathers data, and implements care plans under supervision of licensed medical staff. Responsibilities include accurate clinical documentation, maintaining patient records, preparing patients for exams, administering medications and vaccines, managing medical supplies, and ensuring compliance with healthcare regulations. Key Duties: Provides patient education, follows up on lab results and prescriptions, answers patient and provider communications, and assists with insurance pre-authorizations. Builds collaborative relationships within a multidisciplinary team to support patient care. Qualifications & Skills: Licensed Practical Nurse with at least one year of experience preferred. Must hold a Virginia LPN license and CPR/BLS certification. Demonstrates clinical competency, attention to detail, strong communication skills, and ability to work in a team environment.
