St. Francis Hospice (SFH) is growing, and we need a LPN to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.

Responsibilities

• Accurate assessment of each patient's status, problems and needs
• Collect and analyze data from the patient's medical history, nursing assessments and staff input on an ongoing basis
• Provide PRN visits on patients who call the answering service
• Provide crisis care visits as assigned
• Provide PRN visits as requested by PCC or MD
• Provide care for patients for the entire County
• Work collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life

Requirements

• Current and active Florida (LPN) License required

Qualifications

• Hospice, home health, palliative care, or related experience preferred

Schedule: Monday-Friday, 4 pm - 12 am | $1,500 Bonus
