Tucson, AZ LPN LPN LPN – DCW Intake Coordinator Dependable Health Services – Tucson, AZFull-Time | Field-Based Position | Paid Per Visit Put Your LPN Skills to Work in the Community Dependable Health Services is seeking an organized, compassionate Licensed Practical Nurse (LPN) – DCW Intake Coordinator to support patients receiving Direct Care Worker (DCW) services throughout the Tucson area. This is a full-time, field-based position with per-visit compensation. You'll work directly with patients in their homes, completing new patient intakes, assessments, supervisory visits, and required documentation while helping coordinate the services patients need to remain safely and independently at home. If you're an LPN who enjoys working independently, connecting with patients and families, and using your clinical skills outside of a traditional bedside setting, this could be a great fit. What You'll Do Complete initial intake visits for new patients requiring DCW servicesPerform patient assessments and gather information needed to initiate servicesConduct required supervisory visits for patients receiving DCW careEvaluate patient needs and identify changes that may impact services or the plan of careHelp ensure DCW services are meeting each patient's needsCommunicate with patients, families, DCWs, case managers, and the clinical teamComplete accurate and timely visit documentationReport changes in patient condition, safety concerns, or service needs to the appropriate team memberAssist with coordinating services for new and existing DCW patientsSupport compliance with company policies and applicable documentation requirements What We're Looking For Current Arizona LPN license in good standingHome health, home care, DCW, or community-based care experience preferredStrong assessment and documentation skillsExcellent communication and organizational abilitiesAbility to work independently and manage a full-time visit scheduleComfortable traveling to patient homes throughout the Tucson service areaValid driver's license and reliable transportationDependable, compassionate, and patient-focused Why Join Dependable Health Services?
