Nogales, AZ office office LPN – DCW Intake Coordinator Dependable Health ServicesPosition Type: Full-Time Compensation: Paid Per Visit Use Your Nursing Skills Beyond Traditional Bedside Care Dependable Health Services in Nogales is seeking a dependable and organized Licensed Practical Nurse (LPN) – DCW Intake Coordinator to support patients receiving Direct Care Worker (DCW) services. This is a full-time position with per-visit compensation. The LPN Intake Coordinator will work directly with patients in their homes, completing new patient intakes, assessments, supervisory visits, and required documentation to help ensure patients receive safe, appropriate, and reliable care. What You’ll Do Complete initial intake visits for patients requiring DCW servicesPerform patient assessments and gather information needed to initiate servicesConduct required supervisory visits for patients receiving DCW careAssess patient needs and identify changes that may affect services or the plan of careEnsure DCW services are meeting the needs of patientsCommunicate with patients, families, DCWs, case managers, and members of the clinical teamComplete visit documentation accurately and within required timeframesReport changes in patient condition, safety concerns, or service needs to the appropriate team memberAssist with coordinating services for new and existing DCW patientsHelp ensure documentation and services remain compliant with company and regulatory requirements What We’re Looking For Current LPN licenseHome health, home care, DCW, or community-based care experience preferredStrong assessment and documentation skillsExcellent communication and organizational skillsAbility to work independently and manage a full-time visit scheduleComfortable traveling to patient homes throughout the service areaReliable transportation and valid driver’s licenseDependable, compassionate, and patient-focused Join Dependable Health Services This is a great opportunity for an LPN who enjoys working independently in the community while combining clinical skills with patient intake and care coordination.
