LPN Continuous Care

Responsibilities

• Provide one-to-one crisis care for hospice patients requiring high-intensity, highly engaged clinical supervision.
• Elevate patients’ quality of life and relieve caregiving pressures on loved ones.
• Care for patients in their homes—private residences or assisted living/skilled nursing facilities.
• Work shifts of eight to 12 hours, helping patients remain comfortable while receiving palliative treatment.
• Visit the local program office once a week to meet with an interdisciplinary team (social workers, physicians, chaplains, nurses) for holistic patient and family support.

Requirements

• Active nursing license (LPN).
• At least one year of hospital nursing experience.
• Reliable transportation.
• Demonstrate Commitment, Compassion, and a Can‑do Attitude.
• Keep patient welfare at the center of all actions and extend comfort and dignity to patients at end of life.

Schedule

Shifts are eight to 12 hours in length.
