LPN Continuous Care

About the role

As an LPN with VITAS Intensive Comfort Care® (ICC) service, you will provide one‑to‑one crisis care for hospice patients whose health challenges require high‑intensity, highly engaged clinical supervision for a short period of time. Your compassionate, responsive care will elevate patients’ quality of life and relieve caregiving pressures on their loved ones.

Responsibilities

• Provide one‑to‑one crisis care for hospice patients needing high‑intensity supervision.
• Work in patients’ homes — private residences, assisted living, or skilled nursing facilities.
• Work shifts of eight to twelve hours.
• Visit the local program office once a week to meet with an interdisciplinary team (social workers, physicians, chaplains, nurses) to coordinate holistic patient and family support.

Requirements

• Current nursing license (LPN).
• At least one year of hospital nursing experience.
• Reliable transportation.

Schedule

• Shifts range from eight to twelve hours.
• Weekly visit to the local program office for interdisciplinary team meetings.
