LPN - Community Hospice - Albany/Rensselaer

About the role

The Community Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice Home Care team. This rewarding role involves providing compassionate end-of-life care and support to patients and families in the comfort of their homes in Albany and Rensselaer Counties.

Position Responsibilities

• Provide skilled nursing care to hospice patients in the home setting.
• Monitor patient status and communicate changes to the interdisciplinary team.
• Support patients and families through education, symptom management, and compassionate care.
• Aid in the implementation of the hospice plan of care.
• Collaborate with nurses, physicians, social workers, chaplains, home health aides, and other hospice team members to ensure quality care.

Qualifications

• Current New York State Licensed Practical Nurse (LPN) license.
• One year LPN experience.
• One or two years of homecare and/or Hospice experience preferred.
• Valid driver's license and reliable transportation.
• Strong communication, documentation, and organizational skills.
