LPN Clinical Care Navigator
About the role
As a key member of the Palliative Care team, you will coordinate service inquiries and offer prompt access within the Empath Health Network of Care. Your focus will be on delivering excellence in customer service by being caring, compassionate, empathetic, and responsive to the needs of all who reach out—both internally and externally.
Since 1983, Hospice of Marion County has served families across Ocala and Marion County with compassionate, expert end-of-life care. Responsibilities
- Provide optimum access to the Empath Health Care Continuum via telephone, website, fax, and walk-in inquiries.
- Articulate information and education regarding programs and services in a customized manner appropriate to each customer.
- Facilitate appointments through effective triage to the Empath Health service/program that best meets the patient’s needs.
- Ensure accurate and timely scheduling of patient appointments.
- Work effectively with patients, families, community providers, and team members to gather and input all necessary information for accurate and complete referrals into the patient’s electronic medical record (EMR) in real time.
- Ensure physician certification information and other pre-admission regulatory requirements are received and inputted into the EMR.
- Maintain patient and family confidentiality while allaying fears and concerns through calm, empathetic, and compassionate listening skills.
- Create a comfortable and supportive atmosphere for individuals seeking information, support, and assistance with program details.
- Assist Care Navigators with any clinical needs within the LPN scope of practice.
Requirements
- Associate’s degree (A.A.) or equivalent from a two-year college or technical school; or six months to one year of related experience and/or training; or an equivalent combination of education and experience.
- Maintain an active LPN license in accordance with State of Florida requirements.
- Valid CPR certification from the American Red Cross or American Heart Association.
- Knowledge of HIPAA compliance regulations.
- Basic knowledge of non-hospice related healthcare and community resources.
