LPN Clinical Care Leader

About the role

The LPN Care Coordinator is accountable for assisting the Director of Nursing/Assistant Director of Nursing and Nurse Manager, RN in the management and leadership of a specific nursing unit.

Responsibilities

• Evaluates available resources to assure quality outcomes of care.
• Aids in the direction and implementation of the nursing process.
• Aids the Director of Nursing and RN Nurse Manager with systematic data collection.
• Aids in the development and implementation of care plans.
• Aids in the revision of care plans.
• Carefully coordinates the management of the care of residents through a well-defined care delivery system.

Qualifications

• Current LPN registration in the state of Minnesota.
• Successfully completed or seeking certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
• Ability to communicate verbally and in writing when coordinating the care of the residents.
