To provide quality patient care in collaboration with physicians and registered nurses. The LPN is responsible for assessing and documenting patient findings, collaborating on care planning, and ensuring optimal patient care through communication with healthcare providers, patients, and significant others.

Location: LifeCare Roseau and Warroad Clinic
Shift: Days (Monday through Friday, varies)
Hours: 8:00 AM – 5:00 PM
FTE: 0.8
Weekends/Holidays: No

Responsibilities

  • Assist the registered nurse or licensed practitioner in identifying patient needs, priorities of care, and goals based on patient-specific factors (age, ethnicity, economic status, lifestyle, beliefs).
  • Contribute to evaluating the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions.
  • Modify the plan of care in collaboration with a licensed practitioner.
  • Demonstrate competency in department-specific functions per standards, policies, and procedures.
  • Function as a member of the multidisciplinary team to implement an integrated, patient-centered care plan.
  • Assign or delegate nursing interventions to competent personnel safely.
  • Identify early warning signs of changes in a patient’s condition and report appropriately.
  • Implement nursing interventions and prescribed medical regimens in a timely, safe manner; document care accurately.
  • Collaborate and communicate relevant patient information with patients, families, and multidisciplinary team members to ensure continuity of care.
  • Report patient self-reported symptoms of pain promptly per standard operating procedure.
  • Participate in health education approved by a licensed practitioner.
  • Assist in discharge planning, education, and counseling to optimize patient/family understanding.

Requirements

  • Current active MN LPN license.
  • Acute care experience (preferred).
  • Current BLS certification (preferred).
  • Excellent communication and interpersonal skills.
  • Safe medication administration practices.
  • Ability to care for a diverse patient population with courtesy and respect.
  • Strong teamwork, organizational, decision-making, and time management skills.
  • Excellent clinical skills.

Pay

Pay Range: $22.30–$31.21 per hour. It is not typical for an individual to be hired at or near the top of the range. Compensation decisions depend on factors such as relevant work experience, education, certification, licensure, and internal equity. Hourly pay is one part of the overall compensation package.
