(LPN) Charge Nurse

About the Role

The Charge Nurse is responsible for coordinating and supervising assigned nursing personnel and clinical suites, performing routine and standardized duties to ensure competent nursing staff management. This role coordinates nursing staff according to the philosophy, objectives, and policies/procedures of New Horizon Family Health Services, Inc. (NHFHS). The Charge Nurse also handles daily duties involving direct practitioner support and clinical patient care.

Responsibilities

• Patient Assessment and Care Team Support
  • Collects and documents demographic data and history from the patient, family, and others.
  • Identifies gross manifestations of common diseases and abnormal conditions.
  • Identifies gross states of faulty emotional and mental health adjustment.
  • Obtains and documents vital signs and immediately reports abnormal/emergent results to the practitioner.
  • Supports practitioners in the delivery of medical care, productivity, and patient flow management.
  • Consults with practitioners and other healthcare team members to coordinate the provision of care and patient education in conjunction with other services/care plan activities.
• Supervision
  • Evaluates assigned nursing staff and completes job performance reviews as required.
  • Collects information and prepares necessary reports and performance documentation.
  • Works with the Practice Manager to maintain appropriate clinical staffing coverage.
  • Orders supplies and other resources required for execution of nursing tasks.
  • Works with the NCSSD to coordinate Nursing, Medical Assistant, and Patient Care Technician Orientation to promote competency and quality in nursing duties.
• Quality
  • Implements NHFHS Nursing/Clinical Department policies and procedures.
  • Ensures delivery of quality, safe, efficient, and competent nursing care.
  • Assists in contributing input/information into the nursing personnel budget.
  • Contributes suggestions, ideas, and information to refine nursing policies and procedures.
  • Participates in CQI, internal committees, and special activities that promote improvement of patient flow in the clinical area.
• Community
  • Maintains good public (working) relations throughout the community.
  • Assists, as assigned, with special projects involving community partners and the nursing staff.

Requirements

• Education:
  • Required: LPN with a valid South Carolina Board of Nursing License.
• Experience:
  • Required: At least two years of community nursing experience.
  • Preferred: Minimum of 3 years' experience.
• Other Requirements:
  • Must maintain a current and active Basic Life Support (BLS) certification from an approved provider (American Red Cross or American Heart Association). Certification must be obtained through a hybrid or in-person course.
