LPN Care Coordinator (BHI)

Responsibilities
- Conduct comprehensive assessments of patients' medical, social, and emotional needs
- Provide case management services to ensure continuity of care and appropriate utilization of resources
- Monitor and evaluate patient progress, adjusting care plans as needed
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Oversee patient EMR to ensure that patients receive proper care
- Complete reports to reduce patient gaps and hospitalizations
Requirements
- LPN license required
- Minimum of 2 years of experience in acute care or managed care setting preferred
- Proficient in conducting patient assessments and monitoring vital signs
- Excellent communication and interpersonal skills to effectively collaborate with healthcare professionals
- Experience with Geriatric behavioral health preferred
