Responsibilities

  • Supports the Director of Nursing, Assistant Director of Nursing, and RN Nurse Manager in leading and managing a specific nursing unit
  • Aids in care planning, resource evaluation, and implementation of the nursing process
  • Collects resident data and contributes to assessments such as MDS and RAPS
  • Develops and updates comprehensive care plans
  • Collaborates with the interdisciplinary team to ensure resident needs are met
  • Monitors resident conditions and reports changes to RNs
  • Participates in care conferences
  • Ensures care plans reflect current physician orders
  • Provides nursing care under RN supervision
  • Supports and directs LPNs and nursing assistants
  • Affixes staffing needs
  • Ensures safe, competent care delivery
  • Maintains accurate documentation
  • Communicates care needs effectively
  • Adheres to nursing policies and regulatory requirements
  • Supports quality improvement initiatives

Qualifications

  • Licensure and Certification Requirements: Must hold a current LPN license in Minnesota and complete CPR/emergency care certification; must meet employment eligibility per MN Department of Human Services background check.
  • Communication and Functional Ability: Must be able to effectively communicate in both verbal and written forms and perform all essential job functions with or without reasonable accommodation.
  • Preferred Experience and Knowledge: Experience in healthcare settings like skilled nursing or assisted living is preferred, along with familiarity with Medicare coverage and eligibility.
  • Core Skills and Attributes: Requires strong multitasking, adaptability, emotional intelligence, active listening, and the ability to work effectively in fast-paced, diverse, and evolving environments.
