LPN Care Coordinator (BHI)

Location: Hollywood, FL (In-Office) Schedule: Full-Time; 9:00 am - 5:30 pm Salary: $30.75 an hour- $33.00 an hour About Infinite Medical P.C. Job Summary We are seeking to hire a responsible, patient, and qualified Licensed Practical Nurse to assist with reviewing patient's charts. You will review patient records to ensure that information is accurate, and that records and documents accurately reflect the conditions and treatment requirements of each patient. Responsibilities Conduct comprehensive assessments of patients' medical, social, and emotional needsProvide case management services to ensure continuity of care and appropriate utilization of resourcesMonitor and evaluate patient progress, adjusting care plans as neededCollaborate with interdisciplinary teams to optimize patient outcomesOverseeing patient EMR to ensure that patients receive proper careComplete reports to reduce patient gaps and hospitalizations.
