About the role
Join The Wright Center for Community Health as a Licensed Practical Nurse (LPN) Care Coordinator in a dynamic, mission-driven environment committed to improving community health. This primarily desk-oriented role involves remote coordination via telephone with external agencies to ensure seamless patient care management in a fast-paced outpatient setting.
Responsibilities
- Oversee care for patients transitioning from healthcare facilities, including follow-up calls and coordination with the Primary Care Provider-Team.
- Coordinate care for at least 30 high-risk and rising-risk patients using risk stratification tools and managing registries.
- Arrange follow-up visits and manage patient data on symptoms, medication, and support.
- Perform clinical tasks such as drawing up medications for clinicians, monitoring devices (e.g., Ambulatory Blood Pressure, Continuous Glucose Monitoring, Retinavue for diabetic patients), and ensuring HIPAA confidentiality.
- Handle incoming patient calls, create orders, deliver test results, process medication refills, schedule appointments, and coordinate specialty appointments and tests with outside medical facilities.
- Pursue ongoing professional development.
Requirements
- Active Pennsylvania Licensed Practical Nurse (LPN) license.
- Graduate of an accredited LPN program.
- Basic Life Support (BLS) certification.
- Ability to work as part of a care team with excellent communication skills.
- Commitment to process improvement and quality.
- Meet The Wright Center’s EOS People Analyzer Tool and buy into the EOS® model (strongly preferred).

Qualifications
- Previous experience in a health center setting (a plus).
- Experience with electronic health records, specifically Medent (a plus).

Pay
Starting rate of $30.00 per hour, with an additional $1.50 per hour shift differential for weekends and holidays.
Schedule
- Evenings: One late night per week (12 PM – 8 PM).
- Weekends: One Saturday or Sunday per month (with a weekday off when worked).
- Holidays: One to two holidays per year.
