About the role
Join The Wright Center for Community Health as a Licensed Practical Nurse (LPN) Care Coordinator in a dynamic, mission-driven environment committed to improving the health and welfare of our community. This primarily desk-oriented role involves essential remote coordination via telephone with external agencies to ensure seamless patient care management in a fast-paced, outpatient care setting.
Responsibilities
- Oversee care for patients transitioning from healthcare facilities, including follow-up calls and coordination with the Primary Care Provider-Team.
- Coordinate care for at least 30 high-risk and rising-risk patients, using risk stratification tools and managing registries.
- Arrange follow-up visits and manage patient data on symptoms, medication, and support.
- Perform clinical tasks such as drawing up medications for clinicians to administer and monitoring devices like Ambulatory Blood Pressure, Continuous Glucose Monitoring, and Retinavue for diabetic patients while ensuring HIPAA confidentiality.
- Handle incoming phone calls from patients, create orders, deliver test results, process medication refills, schedule appointments, and coordinate with outside medical facilities for specialty appointments and test orders.
- Pursue ongoing professional development.
Requirements
- Active Pennsylvania Licensed Practical Nurse (LPN) license.
- Graduate of an accredited LPN program.
- Basic Life Support (BLS) certification.
- Mission-oriented with a commitment to representing the enterprise professionally.
- Ability to work as part of a care team with excellent communication skills.
- Commitment to process improvement and quality.
- Meet The Wright Center’s EOS People Analyzer Tool criteria.
- Buy-in and experience working in the EOS® model (strongly preferred).
Qualifications
- Previous experience in a health center setting (a plus).
- Experience with electronic health records, Medent (a plus).
Schedule
- Evenings: one late night per week (12 PM – 8 PM).
- Weekends: one Saturday or Sunday per month (with a weekday off when worked).
- Holidays: one to two holidays per year as part of the rotation.
Pay
Starting rate of $30.00 per hour, with an additional $1.50 per hour shift differential for weekends and holidays.
