LPN Care Coordination

Responsibilities

• Evaluates reports to identify patients in need of preventive care and effectively documents, monitors, and reports on these efforts.
• Makes sure Medicare patients receive their Annual Wellness Visits and sets up a documented process to ensure scheduling.
• Engages in regular huddles and uses communication tools with practices to meet patient care needs.
• Participates in planning, implementing, and collecting data for quality improvement initiatives as directed by the supervisor.
• Collaborates with physicians, office staff, and management to report data and perform analytics reflecting quality metrics.
• Ensures care gaps are met through motivational interviewing, implementation of standard orders and protocols, and patient education.
• Maintains care measures according to standards and documents and reports appropriately.
• Runs reports to identify patients who have not had appointments for up to two years and activates processes to schedule them.

Requirements

• Associate’s Degree or Graduate of a Technical School in a relevant field.
• 1+ position-related experience.
• 2+ Acute Care (hospital) experience.
• Licensed Practical Nurse (LPN).

• Critical thinking skills.
• Ability to solve problems and advocate for patients.

Skills

• Knowledge of clinical quality data integration.
• Experience with quality improvement initiatives.
• Ability to communicate effectively with patients and healthcare providers.

Full time, Day shift (United States of America).

Pay

$20.97 - $38.99 per hour.
