About the role

West River Care is a family-run Home Health care organization serving people in the Northeast who want to live independently, manage their health, or receive Hospice care. We provide assistance with daily activities, skilled care from Home Health aides, nurses, and therapists, and end-of-life care in the comfort of patients' homes.

The nurse plans, organizes, and directs services, emphasizing community health education. The professional nurse collaborates with community resources to meet the needs of individuals and families within their homes and communities.

Responsibilities

• Practice as an LPN under the direction of the RN case manager.
• Communicate changes in patient condition to the RN case manager and/or physician.
• Participate in care coordination, including patient/family meetings, goals of care meetings, interdisciplinary team meetings, and QAPI.
• Arrange and order equipment and supplies related to patient care needs.
• Request refills or reorder medications from the pharmacy and/or physician.
• Prepare clinical notes and update the primary physician as necessary.
• Communicate with the physician regarding patient needs, report changes in condition, and obtain physician’s orders as required.
• Collaborate with the interdisciplinary team.

Requirements

• Graduate of an accredited school of nursing.
• At least one year of hands-on clinical experience as a nurse (in any field of nursing).
• Hospice experience preferred but not required.
• Current state nursing license and CPR certification.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Nursing skills per competency checklist.
• Ability to stand, walk, lift, position, or transfer patients; lift supplies and equipment; and perform reaching, stooping, bending, kneeling, or crouching.
• Visual acuity and hearing sufficient to perform required nursing skills.
• Valid driver’s license, insured automobile in good working order, and willingness to drive in accordance with state/organization requirements.

Pay

Starting at $42/hour.

Schedule

Full-Time, 32-40 hours per week (guaranteed hours). No weekend or holiday shifts required.
