About the role

We’re seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one care in home and school environments. This position offers flexible scheduling, meaningful patient relationships, and the opportunity to make a real difference in the lives of pediatric and adult patients.

Responsibilities

• Provide one-on-one patient care in home and school settings
• Implement and document the plan of care to ensure quality and continuity of care
• Administer prescribed medications, treatments, and therapies
• Perform patient assessments and coordinate care with the clinical team
• Report any changes in client condition to the Clinical Manager and physician
• Promote health, teach, and train family members

Requirements

• Active Licensed Practical Nurse (LPN) license
