About the role

We’re currently seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Tobyhanna, PA. Our skilled nurses provide high-quality care and support to pediatric and adult patients in the comfort and safety of their own homes.

Immediate shift opening: Monday through Friday, flexible hours. Trach/vent rate: $28-$32 per hour.

Responsibilities

• One to one patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
