About the role

As an LPN with Traditional Home Health Care, you are able to give each patient the time and attention they deserve as you care for them in the most ideal setting — the patient's home. Our office is located in Dunmore, PA, and we serve Tunkhannock, PA; Montrose, PA; Clarks Summit, PA and surrounding areas. We do our best to keep each LPN's case load within a sensible geographic territory.

Responsibilities

• Assess patient status and follow plan of care in collaboration with physician, nursing supervisor, patient and family members.
• Administer patient care, manage and evaluate the care plan, and teach the patient in accordance with physician orders.
• Report patient status changes, care, and progress to physician and nursing supervisor on a continuous basis.
• Revise plan of care by consultation with physician.
• Timely and accurately prepare, maintain and submit meticulous documentation as required by Traditional Home Health Care, including all interactions with patients, case conferences, medication changes, time points, and changes to plan of care.
• Act as a liaison and maintain contact with patients, therapist, nursing supervisor, and other members of the health care team.
• Inform patients of other services that are available and make referrals to other community resources as needed.

Requirements

• Current license as a Practical Nurse (LPN) in Pennsylvania.
• Must have current driver's license.
• Ability to travel to case assignments.
• Excellent assessment skills, documentation skills and ability to meet deadlines.
• Ability to work independently with minimal supervision and manage time effectively.
• Strong interpersonal skills.
• Demonstrates good verbal and written communication, and organizational skills.
• Acute care and/or home health experience is preferred.
