Job Summary

Seneca Health Care Center is seeking a Licensed Practical Nurse (LPN) to join their team. This position offers a competitive salary range of $27.54-$32.77 per hour, depending on experience, and includes a Shift Differential for 3 pm - 11 pm and 11 pm - 7 am shifts.

Responsibilities

• Provide nursing care in accordance with resident assessments, care plans, and facility policies
• Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice
• Maintain up-to-date resident care information and communicate pertinent information during shift changes
• Supervise and support Nursing Assistants to ensure quality resident care
• Accurately document care, treatments, observations, and medication administration
• Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties
• Follow infection control, safety, resident rights, and confidentiality standards
• Participate in interdisciplinary care planning and quality improvement initiatives
• Assist with admissions, transfers, and other nursing responsibilities as assigned

Requirements

• A High School Diploma or equivalent required; an Associate's Degree preferred
• A valid NYS Licensed Practical Nurse (LPN) License required
• A current CPR Certification required
• Experience in a skilled nursing facility or with the geriatric population preferred
• Strong clinical, communication, and organizational skills
• Knowledge of medication administration, infection control, and resident care practices
• Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
• A compassionate, resident-centered approach to care
